Clery Act Campus Security Authority (CSA) Reporting Form
The form below is intended for individuals designated as a Campus Security Authority, CSA. Campus Security Authority, CSA is a Clery-specific term that encompasses individuals and organizations associated with an institution that are required by law to report crimes they become aware of if they were alleged to have occurred in a Clery geographic location, such as public areas bordering the campus, or in a designated off-campus building owned or controlled by the university.
All reports should be made as soon as possible and no later than 24 hours after receiving the information. A CSA or reporting party does not need to witness the crime. Under the Clery Act, a crime is “reported” when it is brought to the attention of a campus security authority by a victim, witness, other third party, or even the offender. It does not matter whether or not the individuals involved in the crime or reporting the crime are associated with Fisk Univerisity.
